Information collected on this website

This site collects standard technical information such as pages visited, referring source, approximate location, device type, and interactions with booking buttons. Marketing parameters in the page URL may be stored in your browser so that appointment requests can be attributed to the campaign that referred you.

This website does not collect medical information. Do not submit health details, diagnoses, or treatment history through the website.

Analytics and advertising

Aggregated analytics may be used to understand which pages help patients find the information they need. Advertising measurement may record that a booking button was clicked, without identifying you personally.

Appointment requests

Scheduling is completed on the booking platform operated by Revive Primary Care & Med Spa. Information you enter there is governed by the practice's privacy practices, and any protected health information is handled under HIPAA.

Your choices

You can block cookies or use browser privacy settings to limit analytics. Doing so does not affect your ability to request an appointment.
